    WINTER 2012 Skating Application - Skate With US Group Skating Lessons

    Applications must be submitted by Wednesday December 14, 2011.
    Don't wait until the deadline! Classes fill up quickly!

    Please note that all fields are required in order to process application.

    If you need to submit application for more than one skater, you will have an opportunity to do so by clicking "Submit another application" after this one has been submitted.

    2011/2012 Town Resident Recreation Card ID Number (non-residents enter "n/a"):  
    The skater must provide a current resident ID number in order to receive the resident rates or skate on a session indicated "resident only". Without ID card, non-resident rates apply.

    Please specify the most recent season/year that this skater was enrolled in our program:     

    Class Selection (If a class is full, it will no longer be an option in the list.)
     

    Please note that some sessions may be restricted for residents only (Town ID card required).

    Level to be signed up for:
     

    - If the skater has no formal skating lessons, please select Basic 1, Hockey 1 or Snowplow Sam 1, in accordance with the class they are signing up for.
    - If they have previous lessons elsewhere, select that from the list and let us know in the comments section so that we can determine the level. 

    You will be asked at checkout to agree to the follow terms and conditions:  The Amherst Skating Club, its officers and representatives, shall not be responsible for damage or injuries suffered by skaters, officials or spectators. By submitting class application, you waive any right to a claim against the Amherst Skating Club, its members or officers, for any injuries you may sustain. By signing this application, you indicate that you have read/received the Frequently Asked Questions about the program and understand our policies including the refund policy. Refund requests must submitted in writing. No refunds are given after the first class. Please see "Frequently Asked Questions" for the full written policy.
